Your initial appointment is about getting to know each other so that we can learn about your goals and discuss your treatment options. Occasionally, we will perform treatment on the same day as the consultation. However, a medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- X-Rays (if applicable)
- A list of your current medical prescriptions
- If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
We will also ask that you complete your new patient forms online before your in-office visit. Completing these forms before your visit will help you save time.
About your First Visit
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern before surgery (e.g., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.). In addition, please tell us if you are on any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.). Medical conditions and medications can react to the sedation we may use during your treatment.
X-Rays
If your dentist has taken X-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
